What is the purpose of the donor questionnaire?
Every time you donate, you’ll complete a confidential donor questionnaire. This helps us assess your general health and lifestyle to ensure it is safe for you to donate and safe for a patient to receive your blood.
If necessary, we may ask you not to donate that day — even a mild cold can temporarily defer you. To avoid a wasted trip, contact us if you have any queries.
Please note: The electronic questionnaire on our website is for viewing only. For safety reasons, all official questionnaires must be completed at the clinic.
