With blood safety high on the agenda, WCBS Blood Banks have introduced a Wrong Blood in Tube (WBIT) infographic outlining the prevalence, causes, and prevention of WBIT — a risk that, if undetected, could lead to transfusion delays and ABO-mismatched, potentially life-threatening transfusions.

Wrong blood in tube (WBIT) is a common, preventable error that occurs when a blood sample is taken from the wrong patient or labelled with another patient’s details. This results from failing to correctly identify the patient at phlebotomy or labelling the sample away from the patient’s bedside.

Key strategies to prevent WBIT include performing a second-sample verification, confirming patient identity, and labelling the sample immediately at the bedside after drawing it.

To reduce incorrect blood component transfusions due to WBIT, WCBS has adopted a ‘second sample’ policy, as recommended by SHOT (Serious Hazards of Transfusion).
